Before Your Visit

Patient Forms to Help Us Get to Know You Better.

At Beverly Dermatology & Laser Center, we do everything we can to make our paperwork as simple and quick as possible. In fact, you can even complete it in the comfort of your own home before your visit.
Four days before your appointment, we will text you a link to complete your paperwork in advance. Our simple format typically takes 5-10 minutes on your smartphone and includes your demographic information, a brief clinical history, and even a picture of your insurance card. We will still need to ask a few more questions when you arrive in person, but this step will take care of most your paperwork.
If you prefer paper, you can download all our pre-visit paperwork here. Please download and complete the forms and bring them to your visit. With either approach, we can significantly reduce your total paperwork time in most cases.If you can't download paperwork, no problem, just please allot 10 extra min prior to your appointment and we will give you the paperwork at the office.

**Please note: if using Google Maps, please be sure to use Aliso Viejo as the city. Sometimes it defaults to Laguna Niguel and may send you to the Target center, which is incorrect.
Parking Information
There is ample free parking available all around the building.

Additional Information
For routine medical problems, our trained back office staff will answer all of your questions. However, when necessary, they will consult with the doctor and return your phone call to relay his advice to you. Please allow 24 hours for messages left for prescription refills.

What To Expect

Providing you with a comfortable, efficient and complete dermatology exam is very important to you and to us. You can help us help you address your skin care concerns and achieve your skin care goals by doing a few simple things before your visit:
  • Conduct a self exam to identify any moles that concern you or have recently changed.
  • Write down several questions to ask your dermatologist or allergist.
  • Complete your paperwork in advance online. If you cannot complete it online, please arrive 15 - 30 minutes early to complete paperwork.

If you have any concerns please feel to ask any member of our team. Our goal is for you to feel as comfortable as possible, and get all of your questions answered before you leave.

Before Your Visit

How to prepare for your visit

Before you come and see us, make notes of anything that you have concern over. Feel free to ask about what skin care regime or even what sunscreens would work best for your skin type. No question is too small, we are here to help. We want to make sure that you leave our office with complete confidence.

Please arrive 10-15 min early on your initial visit to complete all new patient forms and get settled in. Please also make sure to bring your insurance card and a photo ID.

Surgical procedures are usually scheduled for later dates so that we can plan the proper time as needed and find a day / time that works best with your schedule.
As a courtesy, please call at least 24 hours in advance if you are unable to keep your appointment time. For surgical and medical procedures, there is a cancelation fee if your appointment is not canceled within 24 hours prior.
